total levels required = ⌈ $\frac{6}{4}$ ⌉ = 2

total decoders required of size 4 x 16 to generate 6 x 64 = $\frac{64}{16^{1}} + \frac{64}{16^{2}} = 4 + 1 = 5$

 

but in last level 12 o/p of 4 x 16 are wasting... Therefore u can make one 6 x 64 with the help of four 4 x 16 and one 2 x 4.
