calculation of average access time of cache COA

Question

1) $T_{avg}=H\times T_{c}+(1-H)\times (T_{c}+T_{m})$

2)$T_{avg}=H\times T_{c}+(1-H)\times T_{h}$

My question is when to use these two formulas.......

and can anybody provide me some references for solving secondary storage, IO interface and Interrupts concepts and problems (pdf or link anything....)

If anything is not given in the question or the term "Hierarchical" is given, then use formula 1, else use formula 2.
