The numbers in ascending order will be $123, 132, 213, 231, 312, 321.$ Median will be $\frac{213+231}{2}=222$

are the number 111 222 333 not to be considered as they are 3-digits and the question does not say that "all digits must be different" it says that "all the different three-digit numbers"

So is 111, 123, 132, 213 ,222, 231, 312, 321, 333 thus median is 222 is this a wrong approach?

"each of the numeral 1, 2 and 3 exactly once"

doesn't it say the same?
