Yes by default its hierarchial but the term page fault service time includes memory access time [so no need to add 10 with page fault serive time] so correct sol'n is 0.01 * 50 + 0.99 * 10 --> 10.4 ns

No. It should be page_fault_rate(MM_access_time + Page_fault_service_time)... see after accessing MM only you came to know that the page from secondary memory needs to be swapped in. That process of swapping in, making room in MM,deciding on which one to replace is PAGE FAULT SERVICE TIME.
